Letters from physicians, physical therapists, occupational therapists, specialists, psychologists, psychiatrist, etc….

* Recommendations for a service dog
* Why do they feel it is necessary
Script for the service dog - recommending that your child should have a service dog. This should be from your physician or pediatric specialist.
What happens next?
Your information will processed.
You will be notified on any missing information.
A letter will be sent to you regarding your application status.
Grants will be awarded on determined dates.
Grants will not be awarded directly to the family. They will instead be forwarded to the facility training the service dog in the child's name.
